As a cardiothoracic surgery resident, a pair of loupe is necessary in a few procedures, such as harvesting internal thoracic arteries and performing coronary artery anastomosis. Therefore, after comparing a few different brands, Heine was my final decision. There are a few points which lead me towards Heine: 1. Good transparency for light, 2. The magnifying lenses can me moved away when needed, 3. A head light may be installed when necessary, and 4. Longest working distance comparing to others. 
During cardiac surgery, the desired focal distance is rather longer than others and the working field is also larger. Also, prolonged operating time would also be possible; therefore, the weight of the loupe and comfort are extremely important. Personally, I found Heine is light enough and rather comfortable. Moreover, I have the choice of moving away the magnifying lenses whenever I need to. Even when moving the lenses up in the air, the loupe as a whole does not feel nose heavy and was was to manipulate. 
The only down side of this device is that the fit-in glasses distance, which is too close to my eye lashes, otherwise, I found Heine loupe to be suitable for cardiac surgery and can be used in prolonged surgery.
